Band 7 £50,861 - £59,159 per annum
Plus Distant Islands Allowance of £1,406 per annum
37 hours per week

We have an exciting opportunity for a Frailty Practitioner to join an evolving Integrated Front Door team. We are seeking motivated and enthusiastic practitioners to lead on the development of frailty projects within our Integrated Front Door service.
These posts will be based primarily within Hospital @ Home and work alongside colleagues to identify frailty in both the community and inpatient wards to prevent admission and reduce the length of time people require to spend in hospital. There is a vision to expand the service in line with Scottish Government initiatives. Applicants should be committed to providing a high standard of patient care and interact well within the multi-disciplinary Team.
Applicants should have good communication skills, and a commitment to developing change and promote innovation within the front door services. Post holders are required to hold a driving licence.
The ability, drive and commitment to work as part of a team, sometimes at a distance, and to contribute to the local development of primary care and acute services is important.
