NHS Highland is seeking to recruit a Band 6 Community Mental Health Nurse to join the Older Adult Community Mental Health Team (OA CMHT) based at New Craig's Hospital, Inverness.
About the service:
The Community Mental Health Team for Older Adults provides an innovative community-based service focused on delivering high-quality, holistic care to older adults and their families/carers.
The service works closely with integrated health and social care partners to:
- Assess and identify the needs of older people.
- Develop and deliver holistic care plans addressing mental, physical and social needs.
- Implement and evaluate programmes of care across a range of settings.
- Promote wellbeing and reduce health inequalities within a robust clinical governance framework.
The role covers Inverness and surrounding areas and will involve regular community and lone working.
The role:
As a Band 6 Community Mental Health Nurse you will:
- Undertake comprehensive mental health assessments, including risk assessment and management plans.
- Develop, implement and review person-centred care plans in partnership with individuals, families and carers.
- Deliver evidence-based interventions and act as a care coordinator within a multidisciplinary team.
- Work collaboratively with health, social care and third sector partners.
- Contribute to clinical governance, service improvement and maintain high standards of electronic record keeping.
What we’re looking for:
- Registered Mental Health Nurse (Level 1) with current NMC registration.
- Experience in community mental health or relevant clinical setting.
- Strong communication and interpersonal skills.
- Ability to manage a caseload and work autonomously in a community setting.
- Commitment to delivering high-quality, person-centred care.
The post requires a UK driving license and use of own vehicle.
Desirable:
- Experience working with older adults and/or dementia care.
- Knowledge of Stress & Distress.
- Knowledge of local pathways and multi-agency working.
