You will carry out the comprehensive assessment and triaging of patient referrals ensuring that an appropriate care pathway is established for everyone.
Key to this role is the ability to liaise with and support patients, their families, carers and multidisciplinary care teams.
Using your clinical expertise and experience to enable patients to be supported and cared for at home to achieve their optimum level of independence, you will be responsible for coordinating escalation of care to improve patient outcomes and reduce the need for unnecessary emergency admission or readmission to hospital or long-term care. This includes ensuring efficient patient flow between acute care and a wide range of downstream and community services, their aim being to facilitate safe, timely discharge.
You will be responsible for ensuring that requirements from multidisciplinary/ multiagency care teams, external agencies, equipment and support are coordinated to prevent deterioration, to prevent unnecessary admission to hospital and to facilitate discharge, or transfer to another facility.
As capacity allows, you will be expected to support the wider community nursing team, optimising patient care, while maintaining your skills.

We are looking for individuals who demonstrate an energetic and flexible approach to their work, and who have previous experience of working in community.
The ability to work well within a team is essential as well as the ability to work without direct supervision.
You will need to have excellent verbal and non-verbal communication skills and be able to motivate service users.
You should have a genuine interest in community working and the diversity it brings. We are looking for leadership qualities and excellent communication skills.
You should have a wide range of clinical knowledge and skills and evidence of a continuous professional development.
You must be caring, professional, work well in a high-pressure environment and easily adapt to changing situations.
Please note that these positions require the successful applicant to work a variety of shifts, including weekends.
Hours of work are Monday – Sunday, 0800 – 2000hrs
