The Extended Community Care Team forms part of the services provided jointly by Argyll & Bute Community Health Partnership and Council. The main functions and objectives of the Extended Community Care Team service are to: contribute to the delivery of health care/reablement in the community.
Manage and organise a defined caseload providing leadership and work in collaboration with the Integrated Team, wider District Nursing Team, Unscheduled care service and other agencies involved in the delivery of care in the community setting.
Engage in assessment of individual and/or family/ carer care needs in conjunction with the clients and within available resources plan, implement and evaluate health and social care programmes conducive with meeting their needs, referring to other team members and agencies when appropriate.
The post holder will be a Registered General Nurse with a current NMC Registration and will hold a relevant specialist District Nursing qualification
or due to the nature and location of this post applications from candidates with other relevant qualifications will be considered e.g., Emergency Nurse Practitioner, Clinical Examination and Assessment skills.
Driving licence is essential.
Sound knowledge of NHS environment and current policies affecting the health and wellbeing of the population.
