The Role
We are seeking a motivated and forward-thinking physiotherapist to join our Medicine of the Older Adult service. This is an excellent opportunity to contribute to the delivery of person-centred, high-quality rehabilitation for older adults within a well-established multidisciplinary team.
The successful candidate will manage a diverse caseload of patients presenting with frailty, deconditioning, delirium and complex multi-morbidity. You will undertake comprehensive occupational therapy/ Physiotherapy assessment and rehabilitation, delivering targeted, goal-oriented interventions to maximise independence and functional recovery.
You will play a key role in supporting safe, effective and timely discharge in line with Discharge without Delay principles, working closely with patients, families and partners across health and social care. The role requires strong clinical reasoning, risk assessment and decision-making skills within a fast-paced environment.
This post offers the opportunity to develop specialist skills in frailty and rehabilitation, contribute to quality improvement activity, and support the ongoing development of pathways for older people within acute services.
We are looking for an individual with experience in acute care or rehabilitation, with a passion for improving outcomes for older adults.
In this key role, you will:
- To be professionally and legally accountable for all aspects of own work including direct and indirect patient care
- To assess capacity gain valid consent and have the ability to work within a legal framework with patients who lack capacity to consent.
- Delegate appropriate tasks and supervise more junior staff (as per organisational chart) to maximise efficiency and ensure the delivery of best practice this would include day to day allocation and reallocation of staff and tasks to meet ever changing service demands.
- Actively develop, implement and participate in Physiotherapy Department and specialty team in-service training programme to promote own personal development and that of staff.
What You'll Bring:
- Registered Physiotherapist with valid HCPC Registration
- Educated to/working towards/operating at Degree Level
- Post graduate expertise within speciality / area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently.
- Comprehensive knowledge of clinical guidelines and standards within the speciality / area of practice.
- A driving licence is required; this must be a full UK/EU/EEA licence.
