At the heart of this transformation is our newly established Interface Care Division. This exciting division has been created to drive bold innovation, strengthen integration between primary and secondary care, and put patients firmly at the centre of everything we do.
Over the next three years, the division will lead a series of ground-breaking developments.
These include our pioneering FNC+Plus care model, the launch of one of the UK’s largest Virtual Hospital with 1,000 virtual beds and clinics, the introduction of Electronic Triage at our hospital front doors, and the creation of Rapid Assessment & Care (RAaC) units to ensure patients receive timely and seamless support. This is about more than improving services it’s about transforming the way care is experienced. By shifting the balance of care closer to home, enhancing patient pathways, and reducing delays, we will ensure people receive the right care, in the right place, at the right time.
These roles are specifically designed to work towards transforming care for frailty patients and older adults by providing NMAHP led ambulatory care, virtual clinics and remote monitoring. Therefore, experience in working with frailty patients and older adults will be essential. Now is the perfect time to join us. The role:
- Is an expert in Older People’s Services and Frailty and an autonomous practitioner working across all specialties.
- Works across role and organisational boundaries to support the Elderly Care and Medical teams Liaises with and refers to other social and health services to provide comprehensive patient management and optimise patient outcomes.
- This may include risk assessment for complex patients.
- Supports and is proactive in maintaining and improving all aspects of clinical governance, evidence based practice and quality assurance within the specialist area of elderly medicine. Provides assessments and treatment plans for patients across all specialties, including ED and IAU.
- Contributes to the CGA process.
- Regularly undertakes research and audit to develop a service that responds to national initiatives and policies. With the opportunity of presenting findings at national and international conferences.
- Provides expert support and education for health professionals, relatives and carers within and out with the service.
Duration, Location, and Working Pattern:
There are 2x permanent posts of 36 hours per week available.
This post will be based at multiple sites.
Key responsibilities:
- Provides expertise in relation to Older people and Frailty at FNC+
- Works at a very high organisational level of personal and professional autonomy and is able to make decisions where precedents have not previously existed.
- Develop the Frailty Service at FNC+ in conjunction with the Elderly Care Consultants, the medical team and wider MDT.
- Enhance the whole system pathway for frail elderly patients.
- Provides clinical leadership and has responsibility for the development and implementation of evidence-based assessment, treatment and discharge plans for the medicine for the elderly service in collaboration with the multi-professional team.
- Acts as an expert clinical resource with expert knowledge and provides education, direction and training to health care staff, patients and carers both within the immediate service area and across GG&C.
- Is responsible for and manages a highly complex patient case load where patients frequently present with complex co-morbidities.
Knowledge, training, qualifications and/or experience required to do the job:
AFPs are educated at Masters Level in advanced practice and are assessed as competent in this level of practice. As a clinical leader they have the freedom and authority to act and accept the responsibility and accountability for those actions. This level of practice is characterised by high level autonomous decision making, including assessment, diagnosis, treatment including prescribing, of patients with complex multi-dimensional problems Registered Nurse on the NMC Register or The Health and Care Professionals Council (HCPC).
An appropriate Masters level qualification in Advanced Practice which has enabled competence in:
- Clinical assessment
- Clinical reasoning, judgement and diagnostic decision making
- Anatomy and pathophysiology
- Non-medical prescribing- V300*
- Leading, delivering and evaluating care
- Practice learning/transferable work based learning Significant comprehensive post registration experience within a healthcare setting some of which must be at a senior practitioner level
