About the role
Home First supports people to return home or remain in a homely setting, working in partnership with acute and community teams. The service focuses on prevention, reablement, and supporting people to manage their own health and wellbeing.
You will work as part of a multidisciplinary team alongside Registered Nurses and other health and social care professionals, delivering high-quality, person-centred care in people’s homes.
Locations
Depending on service need you will support services across the following localities:
- Berwickshire (Knoll, Duns)
- Eildon Central (Sime Place, Galashiels)
- Cheviot (Kelso Health Centre)
- Teviot (Hawick Community Hospital)
- Tweeddale (Hay Lodge Community Hospital)
What we’re looking for
- Experience as a Healthcare Support Worker
- Confident working independently and as part of a team
- Flexible and adaptable to meet service needs across localities
Essential requirements
- NPA Level 6 or SVQ Level 3 in Healthcare (or equivalent)
- Full UK driving licence with business insurance
- Ability to work shifts between 08:00 and 20:30, 7 days a week
